It is sometimes referred to as CG 1258. “This is a statement of service by an American citizen aboard a vessel that is recorded under United States legal requirements.

A Certificate of Identity from the United States Coast Guard or a Certificate of Nationality from the owner’s home country is necessary if the vessel will be registered in the United States by non-U.S. citizens according to archives.gov.

In order to prove that it is allowed for any non-citizen who presently has ownership of a vessel to give work to any U.S. citizen for them to be compensated for their time spent aboard the boat, this document is being written.
